PARTICIPATION AGREEMENT AND RELEASE

Voluntary Participation.
Player has voluntarily applied to MVP Baseball for instruction, counseling and training, with respect to the game of baseball. Player has significant prior experience with said game and is aware that participants in said game are exposed to possibility of personal injury.

Assumption of Risk
Player is aware that Baseball is a hazardous activity. He or she is participating in the activities of MVP Baseball with knowledge of the danger involved, and hereby agrees to accept any and all risks of injury or death which may result from participating.

Release
As consideration for being permitted by MVP Baseball to participate in their activities and the use of required facilities, player hereby agrees, on his/her behalf of his/her assigns, heirs, distributes, guardians and legal representative that player will not make a claim against, sue or attach the property of MVP Baseball, its employees, agents, servants or others who assist with the activities of MVP Baseball. Player hereby releases MVP Baseball and its employees, agents, servants or others who assist with the activities of MVP Baseball. This release applies to all individuals, successors and companies which are related in any way to MVP Baseball.

Medical Authorization
Should player need medical attention for any reason, player and parent/guardian hereby authorize MVP to seek that medical treatment that MVP deems appropriate at player and parent/guardian's expense.

Authorization to Use Name or Picture
Player hereby permanently and irrevocably authorizes MVP Baseball to use his/her name, biographical information and/ or picture on MVP Baseball's web site, videos produced by MVP Baseball, written advertisements, mailings and other types of promotion and advertising. The only restriction on the authorization granted in this paragraph shall be the limitations imposed on player by the organizations running Major, Minor or other baseball leagues.

 

Commitment
We all know how committed one needs to be on a team. MVP Baseball Teams are no different than your travel, scout or high school team. After today's event, parents, please talk to your son if he is selected to train or to go to any tournament with MVP Baseball and the player accepts, MVP will need 100% commitment from the parents and player.
Refunds

Players or parents are not eligible for a refund for any reason.

This includes and is not limited to American Legion or Connie Mac playoffs, football practice, vacations, and summer school. Once a player is accepted for a tournament or training and if the player chooses not to participate it will drastically weaken the team and is not fair to those players who have made 100% commitment. All moneys paid will be forfeited if the player or parent cancels, including payments made by credit card, check, and or cash. Credit cards deposits and payments will not be credited back to you.
Players and or parents who have been selected and who have agreed to participate with a MVP Baseball team, tournament, showcase tournament, and or training camp hereby authorize MVP to charge monthly payments and other amounts due against any credit card on file.

If there is an injury, a 25% cancellation fee will be charged. The balance of moneys paid will be credited towards another event. No cash refund or credit card reimbursement will be made. We will need a written letter from you physician regarding injury.

If you have been selected or recruited for one of our international teams, no exceptions will be made for a refund for any reason including injury. If a player is injured and cannot participate, the player will have to go as a spectator. Players or parents who have registered and who have decided they do not want to go will forfeit any monies or deposits paid.
